Brugada syndrome and temperature
نویسنده
چکیده
Since temperature affects permeability, temperature change forces the Na+channel and other channels to modify their functional state. INa+kinetics depends strongly on temperature. (Nagatomo T, Fan Z, Ye B, Tonkovich GS, January CT, Kyle JW, Makielski JC. Temperature dependence of early and late currents in human cardiac wild-type and long Q-T DeltaKPQ Na+ channels. Am J Physiol 1998; 275:H2016-H2024.)Thus, an increase of 10oC increment the voltage or width by a factor of 1.3 to 1.6 and increases the time of opening and the number of times that the channel is opened by a factor of three. Activation and inactivation kinetics for early INa+are twofold faster at higher temperature, and shift activation and steady-state inactivation. Then, the fever is consideredtriggers to PVT/VF in the BrSjointly with other causes that causing modifcations in the degree ofelevation of the J point and of the ST segment. The other factors capable to get worse the ventricular repolarização are:antimalarial agents;tricyclic antidepressants; class IA (ajmaline and procainamide) and 1C (fecainide, pilsicainide) anti-arrhythmic agents; hyperglycemia; nocturnal bradycardia by vagal predominance; alcohol consumption; mental stress and cocaine use.

A case of Brugada Syndrome unmasked by a postoperative febrile state
Abstract Background: Brugada Syndrome (BS) is an inherited ion channelopathy characterized by an electrocardiographic (ECG) pattern of a coved type ST segment elevation in right precordial leads with or without right bundle branch block. Case Presentation: A 23-year old male presented with right lower quadrant abdominal pain. Further evaluation revealed a diagnosis of acute appendicitis. The ...

ST-segment elevation in Brugada syndrome is caused by a shift in the ionic current balance and the creation of a voltage gradient between the epicardium and the endocardium. This ionic mechanism have been shown to be temperature dependent. We describe a 33-year-old man who presented with fever with the dynamic electrocardiographic changes similar to the Brugada syndrome. These electrocardiograp...
متن کاملPerformance of electrophysiologic study in an asymptomatic patient with type 2 intermittent Brugada syndrome: To do or not to do?
Background: Brugada syndrome (BrS) is an inherited channelopathy, which is associated with sudden cardiac death due to rapid polymorphic VT or VF. There is no definite consensus regarding the management of asymptomatic patients. Some experts advocate close follow-up; others propose the programmed stimulation for risk stratification. We aimed to evaluate the benefit of complete atrial and ventri...

Brugada Syndrome is a rare cause of sudden cardiac arrest and has a unique ECG pattern. In fact, with ST-segment elevation down sloping in the right precordial leads (v1-v3), RBBB pattern in lateral leads and J-point elevation is revealed. We must notice and avoid trigger factors of this syndrome during general anesthesia. Patient is a 39 old man who attended to emergency department with sudden...

Brugada Syndrome is a genetic cardiac disease characterized by electrocardiogram changes consisting of an incomplete right bundle branch block, and ST-segment elevations in right precordial leads V1-V3. These patients are at high risk for developing spontaneous arrhythmias that can be fatal. Many factors during general anesthesia, such as medications, temperature changes, and heart rate variati...
